     In this session, we will examine several different designs, materials and orthosis modifications that may be effectively utilized to prevent or minimize diabetic foot complications - from the bare minimum of a heat-molded prefabricated orthosis to a multi-density custom total contact foot orthosis. We will also discuss the interaction and interface between the orthoses, footwear, and hosiery.

    • This presentation will provide a simulation of an ALJ trial, the 3rd and for all intents and purposes, the last effective stage within the Medicare System of appeals. 
    • The trial simulation will identify key elements of the process and procedure of an ALJ setting, from the viewpoints of the plaintiff (prosthetic provider), defendant (Medicare) and the Adjudication Law Judge.

    Expert panel short presentations describing different aspects of Medical Necessity in P&O with a case-study based breakdown at the end and audience Q&A. Medical Necessity reasoning for P&O devices and features is a public funding source (Medicare/Medicaid) requirement and often requested by commercial payers as well. However, many practitioners struggle with how to write medical necessity vs. functional needs or patient desires as reasoning for components or features. The definition of medical necessity is vague enough to require support in interpretation and application. Until passage of the 2018 law that our records are part of the medical record, we were not often required to make robust medical necessity arguments in our clinical notes as we are today. Therefore, more education and support in this area is needed. Likewise, the shift from fee for service to value-based care for custom devices especially, requires a shift from device delivery to provision of healthcare and being part of the healthcare team in all aspects not least of which is communication (records).

Over her 28-year career in P&O she has worked as a clinician in both private and Hanger practices, full time lower limb prosthetics instructor at UW, and owner and chief consultant of McCoy Consulting working with small, independent P&O practices on clinical reasoning documentation strategies and on the payer side as a clinical reviewer.     Daniel Abrahamson is a board-certified prosthetist/orthotist serving at the VA Puget Sound in Seattle Washington. He is currently the National Program Manager for the Mobile Prosthetic and Orthotic Care program (MoPOC), a program that he co-created, focusing on eliminating barriers for the provision of O&P care in the Veterans Health Administration. He spent 11 years as the Assistant Director at the University of Washington’s Department of Rehabilitation Medicine, Prosthetics and Orthotics Program. He has 23 years of experience as a teacher, researcher, and clinical care provider for people with amputations and orthotic needs. He has co-authored 7 peer reviewed publications and is co-author on several patents. He served on the Washington State Health Technologies Assessment Committee, the UW Prosthetics and Orthotics Advisory and Evaluation Committee, and the Br. Tarcissius Prosthetics and Orthotics Training College Nsawam Ghana collaborative learning community. He is a former president of the NW Chapter of the American Academy of Orthotists and Prosthetist. He is currently a mentor to the Chair of the VA OPPCS Hires Task Force and a member of the Washington State Orthotics and Prosthetics Advisory Committee. In 2022 he was honored with both the National Field Leadership Award and the Under Secretary for Health’s Dr. Robert L. Jesse Award for Excellence in Innovation.

    In 1988 Jim Young's world changed after a near fatal motorcycle crash while goimg home on leave from the USAF. After three years of extensive limb salvage attempts the decision to amputate the Left limb above the knee was made. Less than stellar prosthetic care from his first clinician drove Jim to persue a career in the prosthetics profession. Constantly striving to improve his knowledge base Jim sought out innovators, and prosthetic clinicians who literrally wrote the books on lower limb prosthetics. Jim always makes himself availible to serve as a patient model for whoever claimed to have the "best" emeriging technique for socket design, knee or foot developement, (how better to evaluate a concept or design.) Jim's long held habit of attending meetings and conferences whenever possible allowed him to grow a massive network of mentors and colleages from around the globe, that has came in handy when faced with an unusual challenge, or building a world class line up of speakers for a local conference.As an amputee, clinician, and advocte Jim has served on many boards and committes from city and county political efforts, state gubanatorial appointments, to local and national prosthetic and orthotic organizations. Outside interests include traveling with his wife Pam, being a Pawpaw to 4 grandchildren, golfing, fishing, and is a singer/songwriter with an impressive collection of musical instuments, specifically guitars.

    Daniel Gallego Perez, PhD

    Dr. Gallego-Perez is a professor in the departments of Biomedical Engineering and General Surgery, the Edgar C. Hendrickson Chair in Biomedical Engineering and serves as the director of the Center for Advanced Nanotherapeutics in the Gene Therapy Institute. In addition to high-impact publications, his line of research has yielded multiple intellectual property filings and awarded patents, several of which have already been licensed to industry. Gallego-Perez is the lead inventor of the Tissue Nano-Transfection (TNT) technology, for which he was awarded the NIH Director’s New Innovator Award to work on developing TNT-driven therapies for ischemic stroke and peripheral nerve injury. Gallego-Perez was also awarded pilot funding through NIDDK’s Catalyst Award program to work on developing TNT-driven therapies for diabetes and metabolic disorders. He was recently elected as a 2024 Fellow of the American Institute of Medical and Biological Engineering (AIMBE) and was awarded the Early Career Distinguished Scholar Award and the Excellence in Undergraduate Research Mentoring Award from OSU (2024). Additional accolades include the Harrison Faculty Award for Excellence in Engineering Education, the Cellular and Molecular Bioengineering Young Innovator Award, the Distinguished Alumni Award from both his undergraduate alma mater and OSU’s College of Engineering, the Institute for Materials Research Innovation Award, the Lumley Interdisciplinary Research Award, and the Lumley Research Award. Dr. Gallego-Perez also has a strong track record of mentorship, graduating a total of 8 PhD students over the past 9 years, and providing mentorship to several postdoctoral researchers, visiting scholars, visiting professors, and around 50 undergraduate students since 2015. Gallego-Perez received his bachelor’s in Biomedical Engineering from a joint program between EIA University and CES University (Colombia) in his doctorate in Biomedical Engineering from Ohio State. He joined the Ohio State faculty in 2015.

    Dr. Heather Powell is currently a Professor of Materials Science and Engineering and Biomedical Engineering at The Ohio State University. She joined OSU in 2008 after earning a B.S. in Paleobiology and a Ph.D. in Materials Science and Engineering followed by a post-doctoral fellowship at the Shriners Burns Hospital where she began her work in the developing materials for cutaneous injuries and disorders. Her current research interests include hierarchical scaffolding for tissue engineering, engineered skin mechanics, mechanobiology of fibrosis, anti-scar therapy development, and prosthetic design for maintenance of skin health. She was the recipient of the Ralph E. Powe Junior Faculty Enhancement Award from Oak Ridge Associated Universities, The Distinguished Undergraduate Research Mentor Award (OSU) and the Herman Weed Award for Excellence in Teaching (OSU). Her research has been funded by the National Science Foundation, the National Institutes of Health, the Department of Defense, NIH, Veterans Affairs, the National Institute for Disability Independent Living, and Rehabilitation Research, the Musculoskeletal Transplant Foundation, Zimmer Inc., Milliken Healthcare Products LLC, Avita Medical, Access ProMedical and the Shriners Hospitals Research Foundation.

    Insights into Transtibial Prosthetic Socket Design from Expert UK Clinicians’ Digital Records

    This presentation introduces a method to compare rectifications made by experienced prosthetists across a range of patient demographics and limb shapes, to improve understanding of socket design strategies. Based upon 163 pairs of limb surface scans and CAD/CAM sockets, socket designs were shown to vary along a spectrum of TSB and PTB principles, with a complex interaction between some rectifications and others showing independence. Such analysis might enable clinicians to leverage design records to support education, peer support for complex case management, and enhance evidence-based socket design.
