    William Conroy CPO Growing up, I always loved working with my hands. First, it was Legos. Then I graduated to bicycles, then to cars. At the same time, I always knew I wanted to work in the medical field—I just didn’t know how. After earning my general AA, I was looking through Bachelor’s programs at St. Petersburg College and discovered prosthetics. It was the perfect fit: a way to combine my hand skills with a career directly helping people. Fifteen years later, I’m still learning something new every single day. One of the things I love most about this field—patients who teach us, challenge us, and expand our knowledge. And still, after all these years, nothing compares to the moment when a new amputee stands for the first time. The smiles, the tears, the joy—it never gets old. That’s the heart of O&P, and why I feel grateful every day to do this work.

    Cara has over 25 years working in the prosthetics profession. She serves as the Gainesville Practice Manager, Lead Prosthetist, and Residency Director. Cara’s education started with a Mechanical Engineering Degree with a concentration in bioengineering from Kettering University. She is a graduate of The Northwestern University Prosthetics Program. She spent her first eight years in patient care, gaining experience in technical fabrication, patient care, and patient education. She then worked for major prosthetics manufacturer in clinical services, research and development, product management and marketing, in the US as well as Australia. Cara has traveled the world working with practitioners on patient care best practices. Cara specializes in using video movement analysis for the orthotic and prosthetic profession. While working with clinicians globally, she found video to be a common language that she could use to show others her clinical decision process. This work now helps to show and quantify the care we provide to our patients and their physicians. Cara also received a Master’s Degree in Education, which helps to serve patients and the community by providing educational content that is fun and interactive.

    Expert panel short presentations describing different aspects of Medical Necessity in P&O with a case-study based breakdown at the end and audience Q&A. Medical Necessity reasoning for P&O devices and features is a public funding source (Medicare/Medicaid) requirement and often requested by commercial payers as well. However, many practitioners struggle with how to write medical necessity vs. functional needs or patient desires as reasoning for components or features. The definition of medical necessity is vague enough to require support in interpretation and application. Until passage of the 2018 law that our records are part of the medical record, we were not often required to make robust medical necessity arguments in our clinical notes as we are today. Therefore, more education and support in this area is needed. Likewise, the shift from fee for service to value-based care for custom devices especially, requires a shift from device delivery to provision of healthcare and being part of the healthcare team in all aspects not least of which is communication (records).

    Learning Objective: 

    The audience will recognize the difference between medical necessity and functional needs or desires

    Participants will gain a basic understanding of what type of medical record information can inform medical necessity for an expected health outcome after provision of the prosthesis or orthosis

    Participants will learn a method for processing medical information so it can be applied to medical necessity-based reasoning for prosthetic and orthotic care

    Molly McCoy is a CPO, licensed in Washington state and graduate of University of Washington P&O program. Over her 28-year career in P&O she has worked as a clinician in both private and Hanger practices, full time lower limb prosthetics instructor at UW, and owner and chief consultant of McCoy Consulting working with small, independent P&O practices on clinical reasoning documentation strategies and on the payer side as a clinical reviewer. Molly has worked at Hanger in the department of Clinical and Scientific Affairs since 2018, supporting clinicians and management to communicate clinical reasoning effectively across the US healthcare system.
